+ What are the hours of operation?
The health center is open from 7:30 a.m. to 4 p.m. Monday through Friday excluding holidays, and closed for lunch 1 p.m. to 1:30 p.m.
+ Who is eligible to use the center?
All Maricopa County and State of Arizona employees and their dependents ages 18 months and up.
+ How do I schedule an appointment?
You can schedule an appointment online or call 480-347-4791.
If you are experiencing a medical emergency please call 9-1-1.
+ Are appointments required or can I walk in?
The center will be available by appointment. Same day and walk-in appointments will be accommodated, however appointments are encouraged to reduce possible wait time.
+ What services does the health center provide?
The Premise Health Care Center is your first stop for a diagnosis, routine primary care, prescription refill, annual physical, immunization, or lab work. The center is equipped to handle a variety of personal medical needs including:
o Sick visits including cold, cough, flu
o Stomach ache, earache, urinary tract infections
o Lab draws – including outside provider orders
o Strep and flu tests
o Flu shots, immunizations
o Administration of allergy injections from outside provider orders
o Annual physicals
o School, sport and camp physicals

+ What medical plans are accepted?
Our pharmacy and health center are in network with the following plans:
Maricopa County Medical Plan
Note: Preventive care visits are $0 (FREE)
o Cigna HMO - $10 co-pay
o UnitedHealthcare HDHP with HSA – 15% after deductible
o UnitedHealthcare PPO - $20 co-pay
o Pharmacy Benefit Plan – Catamaran see benefits website
State of Arizona
o Aetna - PPO, EPO, Medicare Managed Care Choice Plan, Traditional Plan
o BCBSAZ - Indemnity, PPO, HMO, Medicare Supplement-Senior Preferred

+ If I do not have insurance through Maricopa County or State of Arizona, can I still use the center?
Yes, those that are covered on a different health plan will be required to file the claim direcly with their health insurance carrier.
+ Can I do lab/blood test orders from another provider at the center?
Yes, the center can perform blood draws ordered by an outside provider. Please bring your lab orders with you to your appointment.
+ Is the center an approved provider for workers’ compensation claims? If I get hurt on the job, can/should I go to the center?
The center will offer basic first aid as well as emergency care in certain situations. The center is not an approved provider for workers’ compensation claims. If you are experiencing a medical emergency please call 9-1-1.
+ Can I make an appointment during working hours?
Yes, the center is open from 7:30 a.m. to 4 p.m. Monday through Friday, excluding holidays. Please work with your supervisor to ensure proper notice/coverage for your position.
+ Do I have to use PTO for an appointment at the center?
+ Can I come in and use the center on my day off?
+ Who manages the center?
Premise Health operates the center.
+ Is my health data secure and private? Will my employer have access to my medical records?
Your privacy is of the utmost importance. Your personal health information will be protected in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Privacy and Security Rules.
Providers working in the center will have access to your medical records. Select team members of the center will also have access to certain information as part of the normal data and claims process.
Due to the sensitive nature of this information, team members with access to center data will undergo additional privacy training and will be required to attest to confidentially requirements. They will only be authorized to access the minimum information necessary to perform their job duties. Access to health center patient information will be audited and any unauthorized access will be grounds for disciplinary action up to and including termination.
+ Am I required to use the center?
No, the center is offered as on onsite convenience for our employees and dependents.
+ Will I have to change my doctor because of the center?
No, the center is offered as a benefit to Maricopa County and State of Arizona employees and dependents as a convenience. You are not required to discontinue services with any of your current/future outside providers.
